ABSTRACT

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is the most common inherited cardiomyopathy and poses major diagnostic and therapeutic challenges in Latin America, owing to heterogeneity in resource availability, access to advanced technologies, and the organization of health systems.

OBJECTIVES: To develop the first Inter-American Consensus on HCM, in order to standardize the care of these patients tailored to the regional context.

METHODS: The Inter-American Society of Cardiology (SIAC) convened a committee of experts from different countries across the Americas. A systematic review of the evidence was performed, followed by the development of recommendations, which were validated using a formal Delphi methodology; consensus was defined as agreement among more than 80% of participants.

RESULTS: The consensus provides a comprehensive approach to the evaluation and management of HCM. It covers epidemiology, pathophysiology, multimodality diagnostic tools, genetics and family counseling, phenocopies, pharmacological and invasive treatment, associated complications, sudden cardiac death risk assessment, special populations (pediatric and pregnant patients), physical exercise, and follow-up strategies. In addition, it includes specific recommendations aimed at facilitating the implementation of these across Latin America.

CONCLUSIONS: The Inter-American Consensus on HCM provides recommendations for the diagnosis and treatment of this condition. It integrates the best available evidence with regional clinical experience and takes into account the epidemiological, economic, and healthcare characteristics of the region. It constitutes a reference framework for standardizing the care of patients with HCM and their families throughout Latin America.
